As is my wont, I am thinking out loud as I try to arrive at an answer to a question that I need answering.
Knowing as I do that a new medication that has been prescribed for Mum does not agree with her as demonstrated by her not behaving as her eusual self and the change in behaviour has been witnessed by more than myself and includes her regular carers, should I:
go with her philosophy and my gut feel and stop the medication
consult the GP who will likely not have a clue as the medication has been prescribed by a hospital consultant AND the hospital consultant has given incomplete/incorrect information on the discharge summary - this despite us asking them to correct the information to accurately reflect what happened and what they did
discuss with GP that we would like to stop the medication and why and then do it anyway after the consultation so it is on record
It is our opinion based on what happened during the inpatient stay that the condition was not properly understood or diagnosed and a generic medication to treat the symptoms and possible cause was prescribed. Further additional medications were prescribed but not administered during the inpatient stay as we chose not to have them as we said they were not appropriate/required.
Perhaps I need to sleep on it and the answer will come to me.

Ive said before there seems to be standard treatments and that is regarded as it! We are not all the same though.
My own way is not to change anything without Dr/consultant approval but thats me! My GP upped my statin but I rang the consultant to check he was happy before taking the increase.
Again ive been lucky- until stroke never took medication so im new to it all - I find your views and those with more experience very interesting and just illustrates we have to look out for ourselves though I’d like to feel part of a team with the professionals - but they do have hundreds to look after so i suppose the individual can be lost in that?

Without knowing the med and reason for prescribing it it is impossible to say. I am with @Baldrick though and would always discuss any med changes with a dr or pharmacist. It may be that it needs tapering off rather than just stopping.

I am always wary of just stopping any medication, so I’d discuss it with your GP first. Is the medication required for something specific? Is there an alternative medication? Can the consultant be asked about this directly?
